What it means
Our interpretation.
The title is the Slayer's legend as Hell tells it: the monsters are the ones who tremble. It is also the soundtrack's pitch to the player, since Doom Eternal's grind of aggression, chainsaw fuel and glory kills is built so the player feels like the thing in the dark that demons are afraid of. Fans pass it around as a motivational line as much as a track name.

'The Only Thing They Fear Is You' is the title of a Mick Gordon track from the Doom Eternal soundtrack (2020), the game where the Doom Slayer fights to end Hell's consumption of Earth. Gordon came back as composer after Doom (2016) and put out a call in January 2019 for 'metal screamers' to join the choir, which included Static-X's Tony Campos and Helstar's James Rivera. The title speaks to the Slayer's reputation: the Doom Wiki marks the demon Prince Ahzrak as one of the few in Hell who does not fear him. Gordon and id Software later severed their working relationship.
