Our interpretation.
Here the speaker demands aspirin with melodramatic force, insisting they'll die on the spot if none turns up. The humour comes from the huge overreaction to a small need, delivered as if it were a matter of life and death, with an ordinary painkiller treated as the only thing standing between them and doom.
